What we do...
We assist people in various transitions throughout their life, whether it be learning life and community skills to move to an independent or semi-independent living situation or maximizing and maintaining skills in supervised living environments. Our flexible staffing allows us to provide the supports when and where they are needed.
We currently support children and adults with developmental needs to include mental retardation, autism, cerebral palsy, epilepsy and traumatic brain injury.
Community Living Options
COMMUNITY BASED RESIDENTIAL FACILITY (CBRF): Agape operates licensed facilities, the majority in Outagamie County. Group Homes (CBRFs) provide 24 hour supervision. Our facilities are six to eight bed homes in the community that offer comprehensive service options. Some of these are fully wheelchair accessible and offer adaptive equipment.
TREATMENT FOSTER HOME: This home provides supervision and services to children in a caring and supportive environment. Licensed by the State of Wisconsin, the treatment foster home specializes in providing support to children that need services not met in traditional foster homes.
APARTMENT LIVING: Agape has a wide variety of options available to support people in their own apartments and homes. We can provide supervised living environments with overnight support. We also provide daily, weekly or monthly support services based on the individual's needs.
IN-HOME SUPPORT: We serve both children and adults who reside in their own homes. Our services can follow them into the school setting and community. We provide a variety of services based on the needs of the individual and their families.
Service Options
Agape of Appleton, Inc. provides a variety of services to people based on their individual needs.
SUPERVISION: Agape will provide the staff needed to support each person based on their assessed needs, which could include overnight awake supervision.
LEISURE TIME ACTIVITIES: Activities are based on individual interest and choice.
COMMUNITY ACTIVITIES: Individuals are encouraged to assist in planning activities that interest them. We will provide supports necessary to help make these opportunities successful.
FAMILY CONTACTS: We encourage healthy family contact and assist with visits, family support and special events.
HEALTH MONITORING/MEDICAL SERVICES: We monitor health and wellness needs. We assist people in securing medical services.
HOME MANAGEMENT: We will assist and work with individuals in cleaning skills and laundry care; shopping, meal planning and food preparation; as well as organization of these and other daily tasks.
MEDICATIONS: We monitor medication based on the individual's need.
NUTRITION: Guidance and support is provided to encourage proper nutrition, including special diets, meal planning, grocery shopping and weight management.
TRANSPORTATION: Agape will provide training in the use of public transportation or will transport individuals based on their needs.
PERSONAL CARE: Staff are trained to provide needed cares in a respectful manner and to facilitate independence of the individual.
PERSONAL DEVELOPMENT: Our staff will assist individuals in learning and maintaining skills in personal hygiene and dress, problem solving, self direction and personal safety.
INDEPENDENT LIVING SKILLS: This is one of the primary functions and goals of Agape. We will work with persons in learning and maintaining these skills.
