MCO certified in accordance with Medicaid Waiver Standards







Supported Apartment Program (SAP)Agape Supported Apartment Program (SAP)
Adults living in their own community apartment throughout the Fox Valley. Up to 24 hour on-site staff availability.


 




Individuals residing in all Agape programs are offered individualized support services that assist them in achieving their goals toward living independently and improving their quality of life within their community.
